The Hospital Problem in Nursing Homes | Alzheimer's Articles ...

People with late-stage Alzheimer?s must often be cared for in a nursing home. But a new study shows that many nursing home residents with dementia are needlessly moved to the hospital when infections and other medical problems arise, a move that can be particularly stressful for anyone with Alzheimer?s disease.

The new findings, published in The New England Journal of Medicine, showed that nearly one in five residents of nursing homes with advanced dementia were moved to the hospital or another facility during the last 90 days of their lives. In some cases, the patient was moved in the final three days of his or her life, even when such care may not be lifesaving.

?Such patterns of transitions are burdensome, particularly since the overwhelming majority of family members state the main goal of care is comfort,? said study co-author Dr. Joan Teno, professor of health policy and practice at the Warren Alpert Medical School of Brown University and a palliative care physician. ?Similar outcomes could be achieved by keeping these patients in the nursing home setting.?

The authors, among them health economists, note that inefficient financial incentives play a role in the frequent moves. Patients who have been in the hospital for three days, for example, are paid for at a higher rate by government programs than those who remain in the nursing home.

A change in surroundings can be particularly upsetting for anyone with Alzheimer?s disease. New settings can make confusion and disorientation worse for anyone with dementia. Such moves also raise the risk for medical errors, since new doctors and other health care staff are often not familiar with the particular needs of the patient.

Hospital Problem In Nursing Homes

Different policies and ideas about end-of-life care in different parts of the country can also lead to wide variations in how often someone with late-stage dementia is moved, the study found. The likelihood of a move varied from just one in 50 nursing home residents in Alaska to more than one in three patients in Louisiana. Blacks and Hispanics were also more likely than whites to experience burdensome transitions.

The researchers reviewed data from more than 474,000 seniors with advanced Alzheimer?s and related problems who were living in nursing homes during their final months of life. More than 90,000 residents experienced at least one burdensome transition. These included a move during the final three days of life, returning to a different nursing home than the one they lived in before they went to the hospital, or multiple hospitalizations in the last 90 days of life.

In many cases, problems like pneumonia, urinary tract infections or dehydration that prompted the moves could have been easily treated within the nursing home, the researchers found. Other procedures included feeding tube insertions and treatments for bed sores.

Medical crises are difficult and stressful for patients and families, particularly in the final stages of dementia. And some medical care can be lifesaving. But the authors call for better coordination between health care staff and a realignment of payment systems ?to make sure that people are getting the right care in the right place at the right time.?

Families, too, can get involved with end-of-life care through measures like advance directives and ?do not hospitalize? orders for loved ones in nursing homes. That way, nursing home staff can alert family members about impending moves and have a discussion about options with the family.

Politics is a Business. A Big, Broken One. Let's Fix It. - Anil Dash

I'm an idealist. I want all governments to work in an ideal, uncorrupted state. But I'd settle for the governments which I live under to work in a way that were at least a bit more responsive and transparent. But part of the reason that doesn't happen is because most of the people I see interact with government based upon their feelings about various governmental institutions, rather than the facts of how it actually works. So here are a few key truths:

  1. Anybody who says "The Government" did something is ineffective at best and just plain ignorant at worst, because there is no monolithic "government" any more than there is a monolithic "The Media" or "The Business". Knowing, and embracing, complexity is necessary for those of us who'd like to change the system.
  2. Money drives an enormous amount of the actions of elected officials. This is not perceived by most elected officials as corruption, but rather as a simple fact, a fact about which they are neither shocked nor surprised. You cannot shame someone about a fact they readily concede.
  3. The reason money drives many actions of elected officials is because it's used to get votes, mostly through the purchase of advertising. It's not because politicians are trying to get rich. Politicians are already rich; That's why they can run.

But if these simple statements indicate that the current system is broken, how come this is the one area that's obviously broken that most tech entrepreneurs aren't trying to fix?

So We're All Doomed?

When I say the political system is broken, it might make it seem like I'm some pessimist decrying that the whole thing is hopeless. But I'm not! Because first, I don't think the process of using our electoral system as a multi-billion dollar media subsidy is going to be sustainable forever.

More importantly, the inescapable motivation for the enormous amounts of money saturating our political and electoral processes is that politicians want votes. It's what lets them become incumbents, a fancy political term that means "ruler for life".

Here's the tricky thing, though: Networks, sometimes, can trump money.

Networks Over Dollars

Now, it's not always the case that enormously vested interests with bottomless pocketbooks can be overcome simply by people banding together through newer, smarter, faster networks. But we've seen it work a few times. Early communities that sprung up around blogging and Craigslist were just trying to meet their own needs, but ended up massively disrupting the wealthy, powerful newspaper and magazine industries largely by accident. You know the same story happened to the industry formerly known as the recording business, too. And those disruptions happened without even trying.

When new technology-based networks are still young, they can be massively disruptive without even intending to be. So what would it look like if we disrupted one of these broken-ass, frequently corrupt, largely inequitable networks on purpose? Well, I can think of no industry in better need of that sort of upheaval than our policymaking infrastructure, at the local, state and federal level. We've let many of the organizations that make up these governmental institutions become unmoored, making many decisions not based on fact or effectiveness, but based on decisions shaped by the money chase that elected officials are obsessed with.

Who's Going To Step Up?

The thing is, there is a ton of opportunity in this disruption that's going to happen. Social networks will reshape electoral politics and the world of policymaking in the next half-decade, and it's just a question of who does it, and on what terms. Even in just the few short years since Expert Labs was formed, we've had to change some of our fundamental assumptions; According to the world we were living in when we started Expert Labs, the widespread, incredibly effective and surprisingly rapid protests against SOPA and PIPA should never have been able to happen. Yet they not only happened, they happened without primarily relying on financial sponsorship of alternate candidates as their primary point of influence.

In short, they used the network to overcome the traditional money-based ways of influencing politics.

The funny thing is, I'm not actually demonizing the fact that money and businesses have a role to play in how the political system works. In fact, as Clay Johnson eloquently explained, we should all do well to be more versed in how political fundraising and policymaking intersect. It's absolutely essential to know the ecosystem around web-based political influence if you want to understand its future.

Going Gaga

Perhaps one of the most overlooked parts of this evolution is that there are going to be new winners. Not just new candidates getting elected to office (although that's great, too!) but new companies which succeed in building thriving new businesses by serving a more responsive, engaged electorate through social networks online. In fact, I'm proud to advise one of the most prominent and promising of them, Votizen, which just got a pretty formidable set of investors who share my optimism that a better political infrastructure is also a good opportunity for building a business that helps make the world better.

I'm not the sort of person who usually ends up advising companies backed by "hot" Silicon Valley investors. (Or Ashton Kutcher. Or Lady Gaga's manager.) But putting aside my own picky preferences about how the tech industry runs, I want this one to work. I want our tech industry to see as much potential, as much excitement, as much glamour, and far more meaning in fixing politics and voting and policy as they do in fixing the way we listen to music or organize our photos.

Because even after Votizen succeeds wildly in getting people to band together to vote more effectively, with more focus on the issues they care about and the facts that impact those issues, we've got a lot of other work to do. We still have to get the smartest, most creative people in our country involved in the hard work of advising policy makers. We have to get regular folks to understand that the drugs that treat their family members' cancer, the highways they drive on to go see their kids' ball games, the parks they go to on the vacation days that they're mandated to have ? all those things are the product of government, even with its current inefficiencies and imperfections. Hell, we have to have every big institution, whether it's government or business or academia or religion, to make itself accessible and malleable by all of us who are affected by their decisions.

Today, though, it's easy to criticize government, or to just complain about it. But bitching about government isn't like bitching about the weather, where we can't do anything about it. In fact it's the opposite ? government is made out of the only thing we really can change: Ourselves. So let's get to work.

It?s been said and proven that pets reduce stress. I can totally attest to that. Just look at how adorably snobbish Tony is. LOL~ He?s one of the most spoiled cats I know and one of the most stubborn too. I wonder where he gets it from.

I spotted him chilling at the veranda so I decided to intrude. :)

What if only one employee closed out the restaurant and left at 11 pm with the bank deposit? . . . You can easily become ?sitting ducks? ripe for accidents, or worse."

The restaurant industry faces another profound security risk, and that is the risk from within. Mr. Cannon encourages restaurant owners to protect their assets, stating, "It's a shocking fact, but 44% of restaurant employees steal from the establishment and 80% of theft is from management personnel. The industry has historically been buried with alcohol and drug abuse. Getting rid of that liability will reduce the likelihood of bigger problems." Cameras can be a good preventative measure for this problem. They also act as a deterrent to would-be embezzlers and to would-be criminals from the outside. Additionally, when there are infractions, they help establish proof for criminal investigations, prosecutions, and other legal matters. Do restaurant customers like cameras though? Cannon believes that most of them do. "It depends on the people, but most customers will feel safer in establishments with cameras and a security system," he says. "If something does happen, they have the confidence that the incident will be recorded and that record will protect them. People are looking for that kind of protection these days."

The Nature of Risk in Value Investing ? Value Stock Guide

Held long enough, the price of a security approximates its value.

  • Buying overvalued stocks in most cases deliver lackluster performance as the value struggles to catch up with the price. The price may decline, or if the business is growing value at a rapid pace, than perhaps there may be some price appreciation. Regardless, there is a better way to invest with less risk and more rewards
  • Buying undervalued stocks, in most cases deliver outstanding performance over time as the price rises to catch up with the value ? as long as the business continues to create value over time

The critical phrase here is ?held long enough?. We will discuss ?risk? with this as a backdrop. For investors who are essentially short term investors, looking for a quick profit, the value in the business does not matter much. When investing is not based on the underlying business fundamentals, it quickly becomes speculation. And speculation is notoriously hard way to create long term wealth on any sustained basis.

So What is Risk?

Quite simply, risk is the possibility that the investment does not work out well. In some cases, you might have to wait longer than expected for the value to be realized. In other cases, you may even experience a loss of capital.

This can happen for a number of reasons:

  • You make a mistake in building the investment thesis for the stock ? Value investing is more art than science. Mistakes can happen. You might over estimate the value of an asset, or under estimate the level of distress on the business. Or you just might have a misplaced decimal point in your spreadsheet. While it is essential to be as thorough as possible during the due diligence period, and lean on the conservative side, it is fool hardy to believe that the possibility of a mistake does not exist.
  • The market continues to stay irrational on the security ? Ever buy a stock that you think is a terrific value, and two years later, the stock is still a terrific value? I have. Although the stock has returned a quite respectable gain in these 2 years, the undervaluation persists. Some of the other stocks may be even less fortunate and the price might barely budge for years, even as the company fundamentals continue to improve.
  • Future may turn out to be different when you reach there ? Value investors try to protect against the uncertain future by paying more attention to the assets and asset quality and less credence to the earnings projections. There are still other ways your investment thesis may trip up. Economic conditions may change. Competition may intensify. FDA approvals may be withheld. Management may turn out to be corrupt.

How do you Protect Against these Risks?

The simplest way to protect against the risks that you may not even know exist, is to demand a healthy discount to value of the security you are buying. Also called ?margin of safety?, a typical 30%-40% discount to the intrinsic value is a good protection against unknowns and your own mistakes in your analysis. In cases where the business has significant risks that are KNOWN, it may still be a terrific investment if you can buy the stock at a greater margin of safety. There are very few sure things in investing and the goal is to maximize your returns adjusted for risk. Sometimes, you may have to take some calculated risks when the potential rewards justify it. To protect your downside, a margin of safety approaching 80% or more may be required (subscribe to my stock newsletter and I will email you an example).

All this assumes that you are researching and analyzing the business of the company and its fundamentals before you make the buy decision. Not knowing the business you are investing in exposes you to another type of risk. The stock may indeed be a great investment, but you may not be able to realize the profits if you miss the red flags that might appear in the future.

Risk is Not the Same as Volatility

If you have done your homework on your stock picks and are very confident in your research, and the company continues to execute in a way that affirms your thesis, why should you care if the stock price continues to go up or down on a daily basis. Daily stock fluctuations merely reflect the daily demand and supply of the shares, which for small cap stocks that are less liquid may be extreme. Have the patience to wait for the market to catch up with the value, which it will over time.

That being said, volatility does create opportunities to increase your stake in the company. If the business is truly undervalued, any opportunity to increase ownership should be welcome.

On the flip side, it is quite possible that a stock may hit your valuation target on a short term upswing. In cases like this, there is no reason to not liquidate your position if you can. Taking profits early is always better. You can redeploy your capital in other more undervalued assets. I always go in a new investment with a target sell price and put in a GTC limit order to sell at that price. Sometimes, I may revise the target price up or down depending on how the company performs in the future. This is not the same as a short term speculation strategy. The difference is that you know what you are doing.

A disciplined buy and sell strategy and the commitment to stick with it is essential to manage your risk. It is altogether too easy to get carried away with ?momentum? and ?hype? when the stock price is on the rise. Emotions and sentiments should play no role in your investing.

With this being said, you should get in the habit of reading Wall Street?s reports and ignoring their recommendations. More on this later!

I made this recipe a week ago, using almost exactly these ingredients. However there are a few very important points to make: 1. If you follow this recipe step-by-step you will need to keep your scallops warm for a long time while making the cauliflower puree. Make the puree first, then keep warm. There's no way it cooks until soft in the time suggested. It took me 20 minutes until it was ready to go into the blender. 2. I needed much more than 1 tablespoon of milk in the puree. I used about 4 tablespoons. 3. The puree needs to be seasoned while cooking. 4. Cut the scallops in half before cooking for 30 seconds on each side. 5. The proportions of oil and vinegar in the vinaigrette should be 1 part vinegar to 3 parts oil. If you made the vinaigrette as decsribed it would be inedible! All that said, the end result is heavenly!

smuggsIf you?re like a lot of families with very young kids (think pre-K), the prospect of a ski vacation is tempting, but overwhelming. Who wants to spend precious vacation dollars taking endless runs on the bunny hill with a toddler while your partner is stuck in the lodge chasing a baby determined eat every French fry she finds on the floor? Family ski resorts like Smuggler?s Notch understand this, and provide age-appropriate programming to help families take (and actually enjoy!) ski vacations with young kids. To put Smuggs? legendary family-friendliness to the test, my husband and I set out from our home in Massachusetts with Homer (3) and Greta (1) for a three-night stay.

Homer was just old enough to enroll in their Discovery Camp (included in most packages), and Greta spent some quality time at the TREASURES child care center (ages six weeks to three). And what did TJ and I do? We skied all day, ate lunch at the lodge, and lounged in the hot tub. Heaven!

The Discovery Dynamos program is for kids aged three to five, and accommodates all ability levels. Homer had already been skiing a few times this winter, so he was bumped up to a group that uses the beginner chair lift, but a lot of the younger groups use Sir Henry?s Learning Hill. This is a nice, wide bunny hill serviced by a magic carpet (it is also where the Burton Riglet Park for beginner snowboarders is located). Also, much to Homer?s delight, the program uses a tractor and wagon to haul kids to the upper slope-side condos, where they can enter the run and ski down without having to use the lifts (easier on the instructors, too!). The camp starts at 9 am, breaks for lunch around 11:30, then goes back out to ski until about 2:30 pm. In the afternoon, the kids come back inside for some indoor entertainment (on Homer?s days they had a science show and a movie). Needless to say, they sleep well at night!

The instructors are friendly, very focused on the kids, and seem to be really excited about reaching a new generation of skiers. If at all possible, I recommend that your child spend a few consecutive days in the program. Homer had the same teacher every day, and became a little more confident and outgoing every day. If your kids are older, keep in mind that there are similar all-day programs for kids up to 15 years of age.

Tip: Though all ski and board ability levels are welcome at the Dynamo level, kids must be potty trained!

While Homer ripped it up with his instructor, Greta spent her days at TREASURES. The facilities are beautiful, and the staff is extremely helpful and accommodating. They even have their own magic carpet lift in the adorable outdoor play area for the older toddlers to try some skiing (refer to the Little Rascals on Snow program)! They specialize in non-recurrent childcare, meaning that they are good at making your child feel comfortable in a new environment, with unfamiliar adults and children. Snacks and meals are provided, but you can bring your own if there?s something special that you know for child will like. They also ask you about mealtimes and naptimes, and really do their best to stick to your normal routine. At the end of the day, you?re given a card detailing everything your child ate, when they slept, and an overall impression of their day.

TREASURES is located right on the trail coming down into the Village Center, so you can easily ski in/ski out to check on your little one. One of the best features of the baby room (0?16 months) is the two-way mirror by the door that allows you peak in without being seen (and thereby causing a meltdown!). But the proof is in the pudding?Greta, who isn?t in any formal, regular childcare at home, had two and a half great days at TREASURES, which equated to TJ and I having two and a half great days of adults-only, guilt-free skiing!

(AP) ? There are super-hot chile varieties. And then there's the sweat-inducing, tear-generating, mouth-on-fire Trinidad Moruga Scorpion.

With a name like that, it's not surprising that months of research by the experts at New Mexico's State University's Chile Pepper Institute have identified the variety as the new hottest pepper on the planet.

The golf ball-sized pepper scored the highest among a handful of chile breeds reputed to be among the hottest in the world. Its mean heat topped more than 1.2 million units on the Scoville heat scale, while fruits from some individual plants reached 2 million heat units.

"You take a bite. It doesn't seem so bad, and then it builds and it builds and it builds. So it is quite nasty," Paul Bosland, a renowned pepper expert and director of the chile institute, said of the pepper's heat.

Researchers were pushed by hot sauce makers, seed producers and others in the spicy foods industry to establish the average heat levels for super-hot varieties in an effort to quash unscientific claims of which peppers are actually the hottest.

That's something that hadn't been done before, Bosland said.

"The question was, could the Chile Pepper Institute establish the benchmark for chile heat?" he said. "Chile heat is a complex thing, and the industry doesn't like to base it on just a single fruit that's a record holder. It's too variable." The academic institute is based at the university's agriculture school and is partially funded by federal grants, as well as some industry groups depending on the project.

The team planted about 125 plants of each variety ? the Trinidad Moruga Scorpion, the Trinidad Scorpion, the 7-pot, the Chocolate 7-pot and the Bhut Jolokia, which was a previous record-holder identified by the institute and certified by Guinness World Records in 2007.

Randomly selected mature fruits from several plants within each variety were harvested, dried and ground to powder. The compounds that produce heat sensation ? the capsaicinoids ? were then extracted and examined.

During harvesting, senior research specialist Danise Coon said she and the two students who were picking the peppers went through about four pairs of latex gloves.

"The capsaicin kept penetrating the latex and soaking into the skin on our hands. That has never happened to me before," she said.

Chile peppers of the same variety can vary in heat depending on environmental conditions. More stress on a plant ? hotter temperatures or less water, for example ? will result in hotter fruit.

The Trinidad Moruga Scorpion's new notoriety is already making waves in the industry and among those who love their hot, spicy foods.

"As with all the previous record holders, there will be a run on seeds and plants," said Jim Duffy, a grower in San Diego who supplied the university with seeds for four of the super-hot varieties. "Like Cabbage Patch dolls right before Christmas or Beanie Babies, it's like the hot item."

Not even Duffy or the researchers would dare to pop a whole Trinidad Moruga Scorpion in their mouths, but there are plenty of videos on social networking sites where heat-loving daredevils have tried.

The blood flow increases and the endorphins start flowing. Their faces turn red, the sweat starts rolling, their eyes and noses water and there's a fiery sensation that spreads across their tongues and down their throats.

"People actually get a crack-like rush," Duffy said. "I know the people who will eat the hottest stuff to get this rush, but they've got to go through the pain."

Pepper experts said there are a handful of people who are crazy enough to subject themselves to the pain, but the rest just want to try out these super-hot peppers on their friends or make killer hot sauce ? and it doesn't take a whole pepper to do that.

More bang for the buck is how Bosland describes it. He said a family could buy two of the super-hot peppers to flavor their meals for an entire week.

The beauty of the peppers is they're not only the hottest in the world, but they're also some of the most flavorful peppers, Duffy said.

"You can make a barbeque sauce or a hot sauce at a mild to medium level using small amounts of these peppers and it will be so darn addictive that you won't want to put your spoon down," he said. "You'll want to eat and eat and eat."

China leader-in-waiting Xi woos and warns U.S (Reuters)

WASHINGTON (Reuters) ? China's leader-in-waiting Xi Jinping wooed and warned the United States on Wednesday, offering deeper cooperation on trade and global troublespots while demanding Washington heed Beijing's demands on Tibet and other contentious "core interests."

Xi, who is almost sure to become China's next president in just over a year from now, laid out his views on ties with the United States in the keynote speech of his visit to Washington.

His message was dominated by reassuring vows about more balanced economic ties and more international cooperation. But he also underscored Beijing's impatience with U.S. policies on Taiwan and Tibet -- issues where many Chinese citizens expect their leaders to show they will stand up to foreign pressure.

"The world is currently undergoing profound changes, and China and the United States face shared challenges and shoulder shared responsibilities in international affairs," Xi told a ballroom crowded with U.S. business executives, academics and policy-makers involved with China.

"We should further use bilateral and multilateral mechanisms to enhance coordination between China and the United States on hotspots, including developments on the Korean peninsula and the Iran nuclear issue," said Xi.

Xi's visit to United States this week has given him a chance to boost his international standing before his likely promotion to head of China's ruling Communist Party later this year and president in early 2013.

Although his speech revealed no new policies, his offer to work with Washington on dealing with Iran's and North Korea's nuclear ambitions could ease U.S. worries that Xi could drive foreign policy in a more hawkish direction.

Yet even as Xi continued his U.S. visit, both sides nudged each other on some of the issues that have fanned friction.

President Barack Obama took aim at China's trade policies, saying he will not stand by when American's competitors "don't play by the rules." And members of Congress pressed Xi on China's detentions of human rights activists.

The Chinese vice president offered his own warnings about Tibet and Taiwan, two territories where Beijing fears that it claims could be undermined by Western pressure.

"History demonstrates that whenever each side handles relatively well the issues bearing on the other side's core and major interests, then Sino-U.S. relations are quite smooth and stable. But when it is the contrary, there are incessant troubles," he said.

Washington should "abide by the one-China policy and take concrete actions to oppose Taiwanese independence," he said.

"We also hope that the United States will truly implement its recognition that Tibet is part of China and its vow to oppose Tibetan independence, acting prudently in issues concerning Tibet," he added.

In early 2010, the Obama administration's decision to move forward with proposed arms sales to Taiwan triggered vehement criticism from Beijing, including warnings of sanctions against U.S. companies involved in the sales.

Those warnings petered out, but Xi made clear that Taiwan remains an acute concern for Beijing's dealings with Washington.

Tensions over Chinese control of Tibet have flared in past months when a succession of protests and self-immolations have exposed volatile discontent. Chinese officials have blamed those tensions on separatists or supporters of the Dalai Lama, the exiled Buddhist leader of the region.

Xi also acknowledged the Obama administration's recent "pivot" toward Asia, but warned it not to push too far.

"China welcomes the United States playing a constructive role in promoting the peace, stability and prosperity of the Asia-Pacific region, and at the same time we hope the U.S. side will truly respect the interests and concerns of countries in the region, including China," said Xi.

Xi, 58, is poised to become China's next leader following a decade in which it has risen to become the world's second largest economy while the United States has fought two wars and endured the deepest and longest recession since the Great Depression that sapped its resources.

Xi met with House of Representatives Speaker John Boehner and Senate Majority Harry Reid on Wednesday morning and after his speech headed to Iowa for the next leg of his trip, which finishes later this week in Los Angeles.

Boehner's office said his staff delivered a letter on the case of Gao Zhisheng, a dissident and human rights lawyer imprisoned in China. The House Speaker also expressed disappointment at China' veto of a U.N. Security Council resolution on Syria.

Many U.S. lawmakers complain that China's yuan currency is significantly undervalued, giving Chinese companies an unfair price advantage that helped lift the U.S. trade deficit with China to a record $295.5 billion in 2011.

Xi said currency reforms already taken by Beijing helped boost U.S. exports to China to more than $100 billion in 2011 and has significantly reduced China's overall trade surplus.

"China has become the United States' fastest growing export market," Xi said. "The trade surplus as a proportion of GDP has been falling from over seven percent to two percent, at a level internationally recognized as reasonable."

U.S. Treasury Secretary Geithner acknowledged on Wednesday that Beijing is gradually letting its currency rise, but not fast enough to please the United States.

"We think they have some ways to go, we would like them to move more quickly," he told a congressional panel.

Xi repeated Beijing call for the United States to eliminate restrictions of exports of high-technology civilian goods, which he said would help bring trade into balance.

Beijing has given the Obama administration a framework for promoting two-way trade and investment, he said.

"Speaking frankly, an important aspect of addressing the imbalance in Chinese-U.S. trade is the United States' own economic policies and structural adjustment," he said.
